How do I create a confirmation booking form?

I’ve recently set up a tribute act and I received my first gig. He wants me to send a confirmation booking form of my act? How do create one? I’ve been crawling all over the web to find a sample form without success. What do I put on it? PLEASE HELP!! Thanks.

CLUES A TALENTS MANAGER OR DIRECTOR IS BAD

Your first clue to the fact that you are talking to the wrong person is if he offers to sign you to a contract without hearing you recite at least a paragraph of acting dialogue. How the heck does he know if you can be an actor if he has never seen and heard you act? Just because you are good-looking doesn’t mean you can be an actor. Acting is not pretending. It is hard work, paying attention to 3 hour acting classes 2-3 times per week, reading plays and books and even harder is the business tasks and hard work you must contribute to get to know all the casting directors who hire talent and somehow land an agent. If you think you can be an actor without even attending one acting class, you are being foolish. Choose a manager who is a member of the Talent Managers Association.

England came from behind to beat Egypt by 3 goals to 1. Egypt went ahead on 23 minutes when Mohamed Zidan capitalized on a slip by Mathew Upson to finish in style beyond Robert Green.
Fabio Capello made a number of substitutions at half time, and it was two of the substitutes, Peter Crouch and Shaun Wright-Phillips, who made the major impact as they made strong claims for place in the England World Cup squad with fine second half performances.
Crouch pounced on Gareth Barrys pass to finish neatly to draw England level on 56 minutes. Wright-Phillips scored on the rebound after Egypt keeper El Hadari could only parry a powerful volley from Milner. Crouch added to his fantastic scoring record at International level to score his 20th goal in 37 internationals, with 10 minutes remaining, when he turned in an excellent cross from Wright-Phillips to make the score line 3-1.
